The Minnesota Board on Aging manages AAAs, ADRCs, and dementia‑capable programs across Minnesota. People with Alzheimer's and their caregivers can find direct support and educational materials on famiyl caregiving, Medicare, housing, transportation, and legal services. It also has links to direct service requests.
Minnesota Board on Aging
Minnesota Aging Pathways (formerly called the Senior LinkAge Line) is a free telephone and chat service designed to make it easy for older adults and their families to find community services. While online options are available, the state suggests calling 800-333-2433 if there is any issue with the forms or chat.

Alzheimer’s Association – Minnesota–North Dakota Chapter

University of Minnesota – St. Louis Park Clinic Memory Care Program
HealthPartners offers evaluation, caregiver support, diagnostics, and treatment of memory and cognitive disorders. The Institute also runs clinical trials covering people with memory disorders, conducts caregiver education interviews, and more. The neuroscience center can be reached at 651-495-6300 for information on services and appointments, or community referrals.

The Center's Health Institute partners with Black communities on health navigation, caregiver support, and culturally grounded wellness, with an emphasis on health and food.
Cultural Wellness Center (Minneapolis)
The ADRC network’s directory helps residents find dementia services, respite, and caregiver supports. The networks website is in the process of moving and updating, so residents may be referred to the Minnesota Aging Pathways service for some care related to memory and aging care.

Headquartered in Saint Paul, CLUES offers bilingual health navigation, caregiver support, and senior programming for Latino families in Minnesota. Its Mayores Mejores program focuses specifically on members of the Latino community who may be experiencing dementia and their caregivers who are looking to learn more about caring and finding referrals to local resources.

The State's health department offers a list of resources on Alzheimer's Disease and Related Dementias, including local supports and tools for caregivers. The "For Caregivers" section has a large number of links, resources, classes, and support groups after the page's informational content.
Minnesota Dept. of Health - Alzheimer's Disease and Related Dementias
United Way 211 provides free and confidential health and human services information for people in Minnesota. The 24/7 service includes links for support groups, memory care and clinics, home healthcare, consultations, and skilled nursing care, among others.

Volunteers of America has a program to support older adults and their caregivers through support groups, health monitoring and education, and respite care. Adult caregivers or informal providers of in-home and community care are eligible for support, so long as they are caring for an adult 60+ years of age or an individual (of any age) with Alzheimer’s Disease and Related Dementias. Caregiver support and education services are free to participants; Healthcare services with VOA's Community Health Workers are eligible for Medical Assistance Reimbursements.

Alzheimer’s Association – Mississippi Chapter
The division coordinates caregiver supports, respite services, and ADRCs across Mississippi. You can use the menu for additional transportation, food, and healthcare support as well as homemaker services. The "Find Services" button will also direct you to local MAC centers, many of which have dementia and Alzheimer's related programming.
